to fill out the order form:
- Fill out an
order form.
- The first
option is the online order form. This form will
appear on the screen in a printable version.
To come back
to this screen, hit the "back" button on your
browser. Go
to the online form now.
- If your
computer does not have a printer you may just write
down the information and send it to us.
- Read the rest
of this page and follow the directions carefully.
What to do next:
- Address an envelope
to and write your return address in the top left-hand corner.
- Double check
that your return address is printed on the order form in
legible printing.
- Place the order
and payment form in the envelope.
- payment
of cheque or money order MUST accompany your order
- shipping
and handling to be billed at cost
- for shipments
outside Canada, Phyto and CITES documentation are available
for an additional $30 (however order cannot exceed $1,200)
- All prices
quoted are in Canadian dollars
- Seal the envelope,
stamp it, and place in a mail box.
- delivery
will take four to six weeks
